Typed claim
The proposal becomes a structure: concept, value, laterality, status. If it cannot be typed, it is not a finding.

Evidence attach
Each stated claim carries a source. No source, no object. This is construction, not a later filter.

Sufficiency
Is this enough information for this consequence? Missing exam → ACQUIRE. Not enough to write as fact → HOLD. Sufficiency as an engine is Open. The question is already the right question.

Consequence
Writing “perforated” is not the same as writing a chat reply. The next reader treats it as examined. Consequence is classified before the write, not after the complaint.

Person
A responsible clinician sees the object. They do not rubber-stamp a paragraph that can still change.

Bind hash
Approval names payload_hash. If the agent mutates the wording after the click, the hash no longer matches. Designed failure: APPROVAL_HASH_MISMATCH. Status: Proposed.

Receipt
The decision is appended, not overwritten. Replay should be possible. Durable audit is Open. In-memory versioning of drafts is Tested.
