Flag vs refuse in a chart.

A warning on a wrong line still leaves the line. Refusal means the finding was never built. The clinician should still see the attempt.

Silence is not a normal eardrum.

If the consult never mentions the left ear, “left tympanic membrane normal” is not caution. It is an invented exam. Models complete. Notes look finished when they are bilateral. The harm is not that the sentence is poorly phrased. The harm is that tomorrow’s reader treats it as yesterday’s otoscopy.

A flag leaves the line. Refusal means the object was never built.

Most stacks flag. A coloured chip. A tooltip. The sentence remains. Flags decay. They are easy to skip in a busy list. The legal record still contains a finding.

Refusal is a different move. A stated claim with no source cannot be constructed. There is no object to file. That is not a filter applied later. It is a constructor that raises.

Refusal can still fail, if it is silent. A dropped attempt teaches no one. The four properties we keep separate: reject, record, surface, preserve. ENT’s research bet is that the clinician sees “Left TM · refused · no source” and the chart does not say perforated, or normal.

We have not shown that this is better for patients. There is no outcome study. There is a prototype that can refuse construction and show the refusal. That is the claim. It is smaller than “safer care.” It is large enough to argue with.
